Job summary

Aveanna Healthcare is seeking a Registered Nurse (Home Health) for a part-time PRN role in the Central and South Brevard county area. The position involves delivering one-on-one home nursing care, performing in-home assessments, and collaborating with physicians to ensure safe, effective care plans.

Ideal candidates hold an active RN license with at least 1 year of clinical experience, are patient-focused, and possess a valid driver’s license with reliable transportation.

Qualifications

• Active RN license in the state of practice with at least 1 year of clinical nursing experience.
• Patient-focused, organized, and motivated to deliver high-quality care.
• Current driver’s license, reliable transportation.
• CPR certification (or ability to obtain).

Responsibilities

• Serve as the primary clinical partner for patients and families, delivering one-on-one nursing care in the home.
• Conduct in-home assessments and translate findings into patient-centered care plans.
• Provide skilled nursing interventions—wound care, infusions, catheter care, medication management, post-operative and chronic disease care.
• Educate and empower patients and caregivers to manage symptoms, medications, and daily care at home.
• Monitor progress, identify changes, and communicate with physicians to adjust plans as needed.
• Coordinate care across providers to ensure continuity and high-quality outcomes.
• Complete timely, accurate documentation reflecting the clinical picture and outcomes.
