Qualifications

• Holds an active Registered Nurse (RN) license in the state of practice with at least one (1) year of clinical nursing experience.
• Is patient-focused, organized, and motivated by delivering high-quality care.
• Has a current driver’s license, reliable transportation, and auto insurance.
• Is CPR certified (or able to obtain certification).

Responsibilities

• Serve as the primary clinical partner for patients delivering individualized care.
• Conduct comprehensive in-home assessments to evaluate patient needs.
• Provide skilled nursing interventions tailored to each patient’s goals.
• Educate patients and caregivers on managing daily care at home.
• Monitor patient progress and communicate effectively with the care team.
• Coordinate care across providers ensuring continuity and high-quality outcomes.
• Complete timely and accurate documentation of clinical care.
