Responsibilities
- Conduct a comprehensive admission assessment and ongoing reassessments.
- Develop, implement and monitor individualized Plan of Care.
- Collaborate with patient, family and healthcare team to ensure quality care.
- Document care activities accurately and timely in patient records.
- Participate in staff orientation and support preceptor/charge nurse duties.
