Job summary

Enhabit Home Health & Hospice is seeking a licensed professional to deliver skilled home health care to patients in their homes. You will teach patients, family members, and the health care team, and perform services according to physician orders and the plan of care (POC).

Ideal candidates hold a state license, have at least one year of licensed experience, and demonstrate basic technology skills including mobile device use. Medicare home health or hospice experience is preferred.

Qualifications

• Must graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
