Home Health: RN/LPN Visit Nurse (SE Metro)

Compensation & Service Area: RN wage range: $85,000-$87,000 annually; LPN wage range: $23-$26/hour plus $41 per visit; Service area includes Hastings, Afton, Farmington, Inver Grove Heights, Cottage Grove, South St. Paul, West St. Paul, St. Paul, and surrounding areas. Mileage reimbursement: $0.70/mile.

Schedule: Full-time, with specific days and hours for RN and LPN roles, including rotation every 6th weekend. Benefits include paid time off, vacation, mileage reimbursement, health benefits, disability insurance, life insurance, 401k with company match, referral bonuses, and career growth opportunities.

Responsibilities:

  1. Conduct initial and ongoing assessments of clients and families to determine care needs.
  2. Develop and revise care plans based on assessments, involving clients and families.
  3. Administer medications and treatments as prescribed.
  4. Provide direct nursing care within legal scope.
  5. Counsel clients and families, provide health instructions, and coordinate discharge planning.
  6. Act as Case Manager when assigned, coordinating patient care.
  7. Maintain documentation, communicate with physicians and community health providers, and ensure compliance with regulations.
  8. Supervise and evaluate home health aides, manage supplies, and organize client appointments and travel.

Qualifications:

  • One to two years of recent acute care experience; home health experience preferred.
  • Experience in customer service or supervising caregivers is a plus.
  • Current Minnesota Nursing License or LPN license.
  • Must pass MVR check.
