Home Health: RN/LPN Visit Nurse - $2500 Sign-On Bonus! (SE Metro)

Join Lifespark as a Home Health: RN/LPN Visit Nurse in the Southeast Metro area. We offer a comprehensive senior health care environment dedicated to helping seniors live independently and healthily.

Position Details

• Wage Range: RN: $85,000-$87,000 annually + $2,500 Sign-On Bonus; LPN: $23-$26/hour + Pay Per Visit at $41
• Service Area: Hastings, Afton, Farmington, Inver Grove Heights, Cottage Grove, South St. Paul, West St. Paul, St. Paul, Rosemount, Eagan, Woodbury & surrounding
• Schedule: Full-time, with shifts including Monday and Friday, 8:00am-5:00pm, with rotation every 6th weekend for RNs; schedule varies for LPNs

Responsibilities

1. Complete initial and ongoing assessments of clients to determine care needs.
2. Develop, implement, and revise care plans in collaboration with clients and families.
3. Administer medications and treatments as prescribed.
4. Provide education and counseling to clients and families.
5. Coordinate care, including discharge planning and case management.
6. Maintain accurate documentation and communicate with physicians and community health providers.
7. Supervise home health aides and manage supplies and scheduling.

Qualifications

• One to two years of recent acute care experience; home health experience preferred.
• Current Minnesota Nursing License or LPN license.
• Ability to pass a Motor Vehicle Record check.
