Job summary

Pathways Healthcare is seeking a Registered Nurse - Home Health for the Westwood, MA area. You will support patients in Newton, Allston, Brookline, Brighton and surrounding areas, Monday to Friday, 8:30 am to 5:00 pm, in a full-time role.

Ideal candidates have 2+ years of nursing experience (geriatrics preferred), a nursing degree, and proficiency with electronic medical records. Training programs and a strong bedside manner are valued, with a focus on compliant, patient-centered care.

Qualifications

• 2+ years nursing experience, preferably geriatrics.
• New nurses encouraged to apply with strong training program.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.

Responsibilities

• Assess and evaluate patient status and write/initiate care plans.
• Regularly re-evaluate patient and family needs.
• Revise the plan of care as necessary.
• Develop care plans based on nursing diagnosis and patient/family input.
• Administer medications and treatments per physician’s order.
• Complete and submit accurate clinical notes of patient care.
