Jethro Health System LLC in Massachusetts is seeking a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to serve as Clinical and Admissions Coordinator for our home health agency. The role focuses on coordinating patient intake and admissions, reviewing clinical information, communicating with the nurse manager and teams, and ensuring compliance with Medicare/Medicaid requirements.

This in-person position offers flexible hours including full-time, part-time, PRN options, and a commitment

Qualificações

• Massachusetts-licensed RN or LPN.
• Graduate from an accredited nursing program.
• Minimum 2 years in home health, medical-surgical, or managed care settings.
• Intake, liaison or discharge planning experience preferred.
• Knowledge of Medicare and Medicaid payers.

Responsabilidades

• Coordinate referrals from hospitals, physicians, other health facilities, community agencies, and families to the clinical team.
• Review eligibility criteria and COP to ensure appropriate referrals.
• Maintain on-call schedule and serve as back-up for on-call.
• Assist with marketing activities as required.
• Liaise with branch office regarding billing, payroll and collections.
• Lead training for new hires and existing staff.
• Track prior approvals for Medicaid and managed care contracts.
