Job summary

Jethro Health System in Massachusetts seeks a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to serve as a Clinical and Admissions Coordinator for our home health agency. The role covers daily operations, patient intake/admissions, and ensuring a smooth pathway for new patients.

A MA license is required. Responsibilities include coordinating referrals, reviewing clinical information, and assisting with staff training and on-call duties.

Qualifications

• Licensed to practice in Massachusetts (LPN or RN).
• Graduate of an accredited LPN program or nursing school.
• Minimum 2 years in home health, medical/surgical, or community health.
• Knowledge of Medicare, Medicaid and other payers.
• Intake/liaison or discharge planning experience preferred.

Responsibilities

• Coordinate referrals between hospitals, physicians, and families and the nursing team.
• Review clinical information to ensure proper admission pathways.
• Prepare on-call schedules and act as back-up for on-call duties.
• Assist with marketing activities and branch liaison with corporate billing.
• Train new hires and existing staff; manage documentation for admissions.
• Maintain records of referrals, admissions, and approvals.
