Job summary

Freedom in Home Services is seeking a Home Health Registered Nurse to provide in-home skilled nursing care in the Omaha, NE area. The role includes conducting initial assessments, developing individualized care plans, and delivering medication management and wound care to clients in their own homes.

The ideal candidate will have an active Nebraska RN license and at least 2 years of nursing experience, preferably in home health.

Qualifications

• Active RN license in Nebraska is required.
• Minimum of 2 years nursing experience, preferably in home health.
• Strong clinical assessment and critical thinking skills.

Responsibilities

• Conduct initial assessments and develop individualized care plans for clients.

• Coordinate with care team to ensure comprehensive, integrated care.
• Educate clients and families on self-care techniques and health maintenance.
• Monitor and document client progress and communicate changes to physicians.
