Job summary

Freedom in Home Services is seeking a Home Health Registered Nurse to provide in-home skilled nursing care in the Omaha, NE area. The role includes conducting initial assessments, developing individualized care plans, and delivering medication management and wound care to clients in their own homes.

The ideal candidate will have an active Nebraska RN license and at least 2 years of nursing experience, preferably in home health.

Job Title: Home Health Registered Nurse

Job Responsibilities

• Conduct initial assessments and develop individualized care plans for clients
• Provide skilled nursing care to clients in their homes, including medication management, wound care, and symptom management
• Coordinate with other members of the care team to ensure comprehensive and integrated care for clients
• Educate clients and their families on proper self-care techniques and health maintenance strategies
• Monitor and document client progress and communicate any changes or concerns to the physician
• Act as an advocate for clients and ensure their needs are met in a timely and respectful manner
• Adhere to all company policies, procedures, and regulatory requirements
• Few evenings of call a month required depending on the position

Job Qualifications
