Zusammenfassung

ProHealth Home Health and Hospice seeks an experienced Registered Nurse to administer skilled nursing care in patients' homes under physician orders and the plan of care. You will work under supervision with the Administrator/Clinical Manager and collaborate with the care team to deliver high-quality in-home services.

The ideal candidate holds an active RN license, a valid driver’s license, and at least two years of nursing experience, with the ability to read, write, and communicate in English.

Qualifikationen

• Active RN license in state or NLC member state.
• Current driver’s license.
• Ability to read, write, and comprehend English.
• Two years nursing experience.
• Support the philosophy and goals of the agency.
• Ability to exercise initiative and work independently.

Aufgaben

• Provides services in accordance with the plan of care.
• Makes initial evaluation visits and reevaluates patient’s nursing needs.
• Initiates the plan of care and necessary steps.
• Provides services requiring specialized nursing skills.
• Initiates preventive and rehabilitative nursing.
• Prepares patient visit notes and care summaries in a timely manner.
