CHRISTUS HomeCare in Lufkin, Texas, is hiring a full-time Home Health Registered Nurse to provide and direct nursing care in patients' homes as prescribed by the physician. The RN will coordinate care with the team, supervise aides, and ensure compliance with laws and agency policies.

Responsibilities include planning and updating care plans with the physician, performing supervisory visits, and documenting care.

Competenze

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License and access to a dependable vehicle.
• CPR certification preferred or ability to obtain within 90 days of hire.
• 1+ years of Home Health experience is preferred.
• Ability to work independently; strong communication, writing, and organizational skills.

Mansioni

• Initiates, develops, implements, and revises the plan of care with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed staff; provides instruction and assigns tasks according to regulations.
• Provides supervisory visits and participates in on-call and weekend rotation as needed.
• Documents visits accurately and timely, completes assessments (OASIS), and maintains infection control standards.
