CHRISTUS HomeCare in Lufkin, Texas is hiring a full-time Home Health Registered Nurse (RN) to join our patient-centered team. You will provide and direct nursing care in patients' homes, coordinate care with physicians and caregivers, and ensure compliance with regulations and agency policies.

Qualificações

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License and access to a dependable vehicle or public transportation.
• CPR Certification or ability to obtain within 90 days of hire.
• 1+ year(s) of Home Health experience preferred.
• Ability to work independently and communicate clearly.
• Solid writing and organizational skills.

Responsabilidades

• Initiates, develops, implements, and revises the plan of care with physician and health care team.
• Supervises home health aides and licensed staff; provides instruction and assigns tasks per regulations.
• Performs supervisory visits and ensures quality care and compliance.
• Delivers high-quality clinical services; conducts accurate OASIS assessments and medication reconciliation.
• Documents visits per policy; coordinates with physicians, patients and families; participates in on-call rotations.
