Job summary

UnitedHealth Group in Gilbert, AZ seeks a Registered Nurse to provide and direct nursing care to patients in their homes under physician guidance and agency policy. You will coordinate the plan of care, supervise home health staff, and ensure high-quality care and accurate documentation.

Required: active RN license in state of practice and a valid driver's license. Preferred: CPR, 1+ year Home Health experience. Compensation includes per-visit and hourly pay with a robust benefits package.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR Certification or ability to complete within 90 days of hire (preferred).
• 1+ years of Home Health experience (preferred).

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed practical nurses, providing instruction and assigning tasks per regulations.
• Documents patient visits per policy and payer requirements, and syncs timely per policy.
• Performs comprehensive assessments (OASIS) including medication reconciliation.
• Communicates with physicians, patients, and family to ensure quality care.
