Job summary

UnitedHealth Group is seeking a Registered Nurse for Home Health to provide and direct nursing care to patients in their homes, coordinating the total plan of care with physicians and other professionals. You will supervise home health aides, perform supervisory visits, and ensure care aligns with state and agency policies.

The role requires an unrestricted RN license, a valid driver's license, and strong communication, writing, and organizational skills.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Solid communication, writing, and organizational skills.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to State and federal regulations.
• Provides required supervisory visits
• Provides high-quality clinical services within the plan of care and infection control standards.
• Completes comprehensive assessments (OASIS) including medication reconciliation accurately.
• Documents patient visits per policy and payer requirements, and syncs timely per policy.
• Communicates information with appropriate agency staff and physicians to ensure quality care.
• Participates in on-call and weekend rotation as needed to meet patient needs.
