Job summary

Beth Israel Lahey Health is seeking an experienced Registered Nurse to lead home health services in the Massachusetts area. You will plan, organize, and direct patient- and family-centered care in home and community settings, aligning with home care regulations and payer guidelines.

You will complete initial assessments, develop and adjust the plan of care, administer medications, and supervise home health aides every two weeks while ensuring HIPAA compliance and accurate OASIS completion in

Qualifications

• Active Massachusetts RN license.
• American Heart Association BLS certification.
• 1 year of direct care nursing experience, preferably in home care/public health.
• OASIS and/or EPIC experience preferred.

Responsibilities

• Complete and document an initial assessment of patient and family to determine home care needs.
• Initiate and revise the plan of care as patient status changes.
• Administer medications and treatments as prescribed by the physician.
• Instruct, supervise, and evaluate the home health aide care provided every two weeks.
• Identify discharge planning needs as part of the care plan development.
• Maintain HIPAA confidentiality and agency policies.
