Job summary

UnitedHealth Group is seeking a Registered Nurse in Home Health to provide and coordinate at-home nursing care in Spokane, WA. You will work with a care team to ensure quality patient outcomes and support families and physicians.

Responsibilities include developing plans of care, supervising aides, completing OASIS assessments, and maintaining thorough documentation while adhering to policies and regulatory requirements.

Qualifications

• RN licensure in Washington required or current RN license
• CPR certification preferred or ability to obtain within 90 days
• Valid driver's license and reliable transportation a must

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals
• Supervises care provided by home health aides and licensed nurses and assigns tasks per state and federal regulations
• Completes comprehensive assessments (OASIS) and medication reconciliation in a timely manner
• Documents visits per policy and payer requirements and coordinates with the care team
• Communicates with physicians, patients, families, and office staff to ensure quality care and positive outcomes
• Participates in on-call and weekend rotations as needed
