Job summary

Sentara Home Care in Suffolk, VA is seeking a qualified Direct Care Registered Nurse to join our Home Health and Hospice team. You will develop and evaluate plans of care, coordinate a patient caseload, and collaborate with a multidisciplinary team to deliver integrated, high-quality care.

The ideal candidate holds an Associate's Degree in Nursing, an active RN license in VA, and BLS certification within 90 days. One year of nursing experience is preferred; Home Care experience is highly valued.
