Job summary

Sentara Home Care is hiring a Direct Care Registered Nurse for home health and hospice in Suffolk, VA. You will develop, implement, and evaluate plans of care, manage a patient caseload, and coordinate with a multidisciplinary team to deliver integrated, high-quality care.

Requirements include an Associate's Degree, an active VA RN license, and BLS within 90 days. One year of nursing experience is preferred, with home care highly valued.

Qualifications

• Valid RN license in state(s) of practice is required.
• Education: Associate's Degree (Required).
• Experience: 1-year minimum nursing experience; Home Care is highly preferred.

Responsibilities
