Job summary

Enhabit Home Health & Hospice is seeking a qualified licensed professional to provide skilled care to patients in the home. You will educate patients and families and collaborate with the health care team to follow the physician’s orders and the established plan of care.

Qualified candidates have a state license, at least one year of experience, and basic technology skills, including mobile device use; valid driver license and reliable transportation are required.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.
• Medication home health or hospice experience is preferred.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
