Qualifications

• One year of clinical RN experience.

Responsibilities

• Create, implement, and coordinate the plan of care to promote wellness and prevent re-hospitalization.
• Perform initial/comprehensive nursing evaluation and develop the care plan per state guidelines.
• Maintain accurate documentation and clinical notes on patient status and interventions.
• Establish and maintain an efficient visit route for home visits.
• Provide high-quality clinical nursing services per state laws and plan of care.
