Job summary

The Beacon Home Health team in Cleveland, TN is seeking a dedicated RN to provide home health nursing care. You will develop and execute the plan of care, coordinate with physicians, patients, and families, and ensure high-quality care from admission to discharge.

This full-time role requires compassionate patient engagement, accurate documentation, and reliable transportation within the service area. Qualifications include one year of clinical RN experience, current TN RN license, CPR

Qualifications

• One year of clinical RN experience.
• Current RN license in the state where services are rendered.
• Current CPR certification, driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.

Responsibilities

• Provide and direct nursing care to patients in their homes as prescribed by the physician.
• Create, implement, execute, and coordinate the plan of care to promote wellness and prevent rehospitalization.
• Maintain and submit accurate documentation and clinical notes from admit through discharge.
• Coordinate information with physicians, agency personnel, patients, families, and community resources.
