Elara Caring is seeking a Registered Nurse Home Health to deliver compassionate, patient-centered care in homes across Valparaiso, IN. You will assess patient needs, develop and adjust plans of care, and coordinate with the care team to ensure safety and quality.

The role requires an active RN license, 1+ year of clinical experience, and a willingness to travel between patient homes with a valid driver's license. Home health experience is preferred.

Qualifikationen

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to lift up to 50 pounds and travel between patient homes; valid driver's license or reliable transportation.

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.
• Providing nursing care according to the plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.
• Educating patients and families on their care and safety.
