Job summary

Elara Caring in Indiana is seeking a registered nurse home health to join our team. You will provide compassionate, patient-centered care in patients' homes, performing assessments, executing plans of care, and coordinating with the care team.

The role requires an active RN license and at least one year of clinical experience; home health preferred. You must be able to lift up to 50 pounds and travel between patient homes using your own vehicle.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate, Diploma, Associates or Baccalaureate).
• 1+ year of clinical nursing experience.
• Home Health experience preferred, but not required.
• Ability to lift up to 50 pounds and travel between patient homes.

Responsibilities

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.
• Protecting patient and staff privacy in accordance with HIPAA.
• Providing nursing care according to the plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.
• Educating patients and families on their care and safety.
• Taking appropriate action when patient conditions change.
• Helping ensure quality, safe, and continuous patient care.
