Advocate Health is seeking a Home Health Registered Nurse for York, SC, to manage an assigned patient caseload in the home setting. You will ensure timely care, documentation, and adherence to the plan of care and physician orders.

You will collect and evaluate health data, identify high-risk patients, and coordinate with a multidisciplinary team to optimize outcomes and safety for patients across York, SC. BSN is preferred and RN licensure is required in the state of work.

Qualifications

• Graduate from an accredited School of Nursing.
• BSN preferred; BLS required per policy guidelines.
• Current RN license or temporary RN Practitioner license in the work/residence state; if NLC, meet home state licensure requirements.

Responsabilités

• Manage daily patient caseload in home health with timely care and documentation per plan of care.
• Assess data from home visits, telephonic contacts, and EMR to identify needs.
• Coordinate care with multidisciplinary team and supervise HHAs/LPNs as needed.
• Implement plan of care and monitor progress toward patient goals.
