Job summary

PeaceHealth is seeking an RN Home Health for a Full Time, 1.00 FTE, Day position. The salary range for this job opening at PeaceHealth is $52.95 – $80.09.

The hiring rate is dependent upon education, training, work experience, and applicable collective bargaining agreements. Hiring bonus may be available and benefits include medical, dental, vision, retirement, PTO, life and disability insurance, and tuition reimbursement.

Qualifications

• Active Registered Nurse license in state of practice.
• Bachelor's degree in nursing preferred.
• Basic computer skills including MS Office applications required.

Responsibilities

• Provide direct skilled nursing care and coordinate care for home health patients.
• Perform initial evaluations and develop/modify the plan of care.
• Complete clinical documentation in compliance with regulations and agency guidelines.
• Coordinate with interdisciplinary team to ensure smooth care transitions.
