Job summary

PeaceHealth seeks an experienced RN for Home Health in a full-time, day shift role. The position includes comprehensive home care planning, patient assessment, and caregiver education as part of an interdisciplinary team.

Required: Nursing degree with RN licensure; 2 years acute or home health experience; BLS. A driving license and insurance are required to operate in the service area. A hiring bonus may be available.

Qualifications

• Accredited School of Nursing required.
• Bachelor of Science in Nursing preferred.
• Active RN license in state of practice.

Responsibilities

• Perform initial patient/home evaluation and determine eligibility for home health services.
• Provide skilled nursing care and education to patients and caregivers per plan of care.
• Conduct physical assessments and develop/manage individualized plans of care.
• Coordinate with interdisciplinary team to ensure smooth care delivery.
• Document accurately and timely per regulatory guidelines.
• Support family Bereavement planning as part of care.
