Job summary

Bronson at Home is seeking a compassionate Registered Nurse to deliver patient-centered care in the home setting. This role supports patients and families, works independently in the field, and collaborates with the Home Health Clinical Services Manager and interdisciplinary team.

You will provide skilled nursing care per physician plans, assess and plan, coordinate with physicians and therapists, delegate tasks to assist staff, and document visits to ensure ongoing patient needs are met.

Qualifications

• Nursing degree and Michigan RN license in good standing.
• BLS certification required.
• Two years of recent acute care experience preferred.

Responsibilities

• Provide skilled nursing care per physician plans and Medicare guidelines.
• Assess, plan, implement, and evaluate patient care.
• Coordinate care with physicians and therapists.
• Delegate tasks to CNAs and support staff.
• Document visits and patient responses.
• Support continuity of care through effective communication.
• Caring for diverse patient populations with compassion.
