Altra Home Health, LLC in Noblesville, IN is seeking a dedicated Registered Nurse (RN) to deliver skilled home health care. You will assess patients in their homes, administer medications, manage chronic conditions, and coordinate with physicians to ensure Medicaid-compliant care.

The role requires an Indiana RN license, CPR certification, and at least 1 year of nursing experience—preferably in home health or Medicaid. A valid driver's license and reliable transportation are essential.

Qualifications

Active Indiana RN license
At least 1 year of nursing experience; home health and Medicaid preferred
Current CPR certification
Valid driver’s license and reliable transportation
Strong communication and organizational skills
Familiarity with electronic documentation systems preferred

Responsabilités

Provide skilled nursing services including wound care and medication administration.
Coordinate care with physicians, case managers, and the interdisciplinary team.
Document care accurately and timely to support Medicaid reimbursement standards.
Educate patients and caregivers on disease processes, medications, and self-care.
Participate in Medicaid audits, quality assurance, and care conferences.
Connect patients with community resources and support services.
