Altra Homecare Group is seeking a dedicated Registered Nurse (RN) to deliver skilled home health services in Indiana. You will provide compassionate, patient-centered care in patients' homes while ensuring compliance with Indiana Medicaid requirements and care plans that promote health and independence.

As part of the care team, you will coordinate with physicians and case managers, educate patients and caregivers, maintain accurate EHR documentation, and participate in audits and quality

Formación

• Current and unrestricted RN license in Indiana.
• At least 1 year of nursing experience; home health and Medicaid experience preferred.
• Solid understanding of Indiana Medicaid policies, documentation standards, and prior authorization processes.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Sharp clinical judgment along with strong communication and organizational skills.
• Familiarity with electronic documentation systems preferred.

Responsabilidades

• Provide skilled nursing services, including wound care, medication administration, chronic disease management, and patient education.
• Ensure all services are medically necessary and properly documented to meet Medicaid reimbursement standards.
• Coordinate care with physicians, case managers, and other members of the interdisciplinary team.
• Empower patients and caregivers with knowledge on disease processes, medication regimens, and self-care strategies.
• Maintain accurate, timely, and compliant documentation within the electronic health record (EHR).
• Take part in Medicaid audits, quality assurance initiatives, and case conferences.
• Champion patient needs and help connect them with community resources and support services.
