Job summary

CareDiem Home Health is seeking compassionate RN nurses to provide skilled home health care in ROCKFORD, IL. You will deliver nursing care under physician orders, perform assessments, medications, and wound care, and monitor patient progress in our Axxess charting system.

This role requires an active Illinois RN license, 1+ year of nursing experience (home health preferred), CPR (BLS), reliable transportation, and strong clinical judgment.

Qualifications

• Must hold an active Illinois RN license.
• Minimum 1 year of nursing experience; home health experience preferred.
• CPR certification (BLS) required; reliable transportation and valid driver’s license.

Responsibilities

• Deliver skilled nursing care under a physician’s order.
• Complete assessments, medication administration, and wound care.
• Monitor and document patient progress using the electronic charting system (Axxess).
• Educate patients and families on health conditions and care.
• Communicate with physicians and the care team.
• Travel to patient homes.
