Job summary

CareDiem Home Health is seeking compassionate RNs to provide skilled home health nursing to patients in Kankakee and surrounding areas. You will deliver care under physician orders, perform assessments, meds administration, wound care, and educate patients and families.

Applicants must hold an Illinois RN license, have 1+ year nursing experience (home health preferred), and CPR (BLS). Reliable transport and valid driver's license required.

Qualifications

• 1+ year nursing experience, preferably home health, is required.
• Active RN license in Illinois is required.
• CPR certification (BLS) required.

Responsibilities

• Deliver skilled nursing care under a physician's order.
• Complete assessments, medication administration, and wound care.
• Monitor and document patient progress using the EHR (Axxess).
• Educate patients and families on health conditions and care.
• Communicate with physicians and the care team.
• Travel to patient homes.
