FAMILY HOME CARE GROUP LLC in Orlando, FL is seeking an experienced Home Health Nurse to deliver compassionate, patient-centered care in patients’ homes. You will develop care plans, educate families, and coordinate services while maintaining regulatory compliance.

The role requires a FL nursing license, 1–2 years of home health experience with OASIS knowledge, and strong organizational and communication skills. Proficiency with EMR systems such as Kinnser/WellSky or Axxess is preferred.

Qualifications

• Current FL nursing license is required.
• 1–2 years of home health nursing experience with knowledge of OASIS.
• Strong organizational, time-management and critical-thinking skills.
• Excellent communication and interpersonal abilities.
• Proficiency with EMR/home health documentation systems (Kinnser/WellSky; Axxess preferred).

Responsibilities

• Develop individualized patient care plans based on assessment findings, physician orders, and patient goals.
• Educate patients and families on disease processes, medication management, safety, and self-care techniques.
• Coordinate multidisciplinary services, including therapy, nursing, and aide support.
• Ensure compliance with documentation standards, agency policies, and regulatory requirements.
• Communicate effectively with physicians, case managers, and other care team members.
• Provide compassionate, patient-centered care in the home setting.
