Job summary

Advocate Health is seeking a Registered Nurse for Home Health in Macon, GA and surrounding areas. The role is full-time, 40 hours per week, with on-call rotation after hours and occasional weekend/holiday duty. A sign-on bonus of up to $3,000 is available for qualified applicants.

The RN will manage patient caseloads in home settings, coordinate care with a multidisciplinary team, and ensure timely documentation and adherence to care plans. BSN preferred; GA RN license required; BLS required.

Qualifications

• Graduate from an accredited School of Nursing. BSN preferred. BLS required per policy guidelines.
• Current RN license in GA or multi-state license.
• Experience in home health nursing or related field is preferred.

Responsibilities

• Manage an assigned patient caseload daily, ensuring timely care and documentation per plan of care and MD orders.
• Collect data from home assessments, telephonic contacts, and EMR; analyze to identify issues and track high-risk patients.
• Coordinate care with multidisciplinary team, supervising LPNs and HHAs to ensure safe, high-quality outcomes.
• Perform treatments and procedures per scope of practice and agency policy; ensure progress toward patient goals.
