Job summary

Adoration Health in Pueblo, CO is seeking a compassionate Home Health Registered Nurse to join our patient-centered team. You will provide expert nursing care in patients' homes, following MD orders and goals.

Responsibilities include creating care plans, administering medications, managing pain, educating families, coordinating with an interdisciplinary team, and maintaining timely documentation. On-call rotation is required.

Qualifications

• RN degree (Associate or Bachelor) with current, unrestricted state license
• Minimum of 1 year nursing practice in patient care; home health, geriatrics preferred
• Valid driver’s license with good driving record and car insurance per Adoration policy
• New grads may be considered in select markets based on program availability
• Current CPR certification

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create home health care plans that align with MD orders and patient goals
• Direct nursing care: administer medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support patient’s family and caregivers
• Collaborate with an interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by the local branch
