Vitalis Healthcare Services LLC in Silver Spring, MD seeks a Home Care Visit Nurse to provide compassionate, personalized care in clients' homes. You will conduct in-home assessments, administer medications, and monitor health status to support independent living.

The role requires a Maryland RN license, at least 1 year of nursing experience (home health preferred), BLS, and reliable transportation. You’ll collaborate with a multidisciplinary team and educate families on care techniques.

Qualifications

• MD RN license required.
• Minimum 1 year nursing experience, preferably in home health.
• BLS certification required.
• Experience with EHR systems preferred.

Responsabilités

• Conduct in-home assessments and develop individualized care plans.
• Administer medications and monitor vital signs.
• Provide wound care, catheter care, and other nursing tasks.
• Educate clients and families on health management.
• Collaborate with multidisciplinary teams to ensure comprehensive care.
• Document patient progress and maintain health records.
• Respond to emergencies and provide immediate care.
• Build strong relationships with clients to enhance comfort and trust.
