Job summary

Classic Home Care in Northville, MI is seeking compassionate Registered Nurses to join our Medicare-certified home health team. You will assess patients, develop and adjust the plan of care with physicians and the interdisciplinary team, and help patients return to independence in their homes.

Qualifications

• Active Michigan RN license
• Reliable transportation
• Valid driver's license and auto insurance
• Home health care experience preferred

Responsibilities

• Provide care to home patients and help them return to independence.
• Conduct initial assessments to create a personalized plan of care with physicians and the interdisciplinary team.
• Monitor patient status and report changes to the physician or health professionals as needed.
• Teach, supervise and counsel patients and families on home nursing procedures.
• Demonstrate critical thinking to improve services and patient outcomes.
