Beyond Faith Homecare is seeking a dedicated Registered Nurse to provide home health care in clients' residences across the region. You will collaborate with an interdisciplinary team, assess patient needs, and implement plans of care to promote healing and independence.

The role requires an RN license, 1 year of clinical experience, and preferred home health exposure. CPR and ongoing professional development are part of the role, with opportunities for growth and flexible scheduling.

Qualifikationen

• Excellent interpersonal and communication abilities.
• Proven decision-making capabilities.
• English reading/writing/comprehension required.

Aufgaben

• Promote and uphold the company's mission, vision, and values.
• Perform initial and ongoing nursing assessments and identify patient needs, including OASIS.
• Monitor and reevaluate clients’ nursing needs and outcomes of care.
• Develop, initiate, and revise the Plan of Care to ensure quality and continuity of care.
• Coordinate care with families, interdisciplinary team, and referring agencies.
• Communicate effectively with clients, families, and health professionals; participate in case conferences.
• Coordinate infection control practices and universal precautions.
• Document timely and accurately to ensure policy compliance.
• Supervise and provide clinical direction to LVN/LPN, HHA, CNA, and other staff.
• Handle on-call duties as assigned and maintain ongoing professional development.
