Job summary

Life Care Home Health Family is seeking an RN to administer nursing care in clients' homes across Texas. You will coordinate with the interdisciplinary team, families, and referral sources to plan and deliver quality care.

Responsibilities include initial assessments, ongoing plan of care, case conferences participation, documentation, and on-call coverage. Requires current RN license, CPR, and valid driver's license.

Qualifications

• One year RN experience in a clinical setting.
• Home health experience preferred.
• Current State RN license in applicable states.
• Current CPR certificate.

Responsibilities

• Promote and exemplify the Company's mission, vision, and values.
• Complete initial and ongoing assessments to identify client needs and plan care.
• Regularly reevaluate nursing needs and outcomes of care.
• Develop, initiate, and revise the Plan of Care for quality and continuity of care.
• Provide effective communication to clients, families, and team members; participate in Case Conferences.
• Coordinate infection control and universal precautions in client care activities.
• Coordinate services and inform physician of changes in patient condition.
• Monitor cases for third-party payer requirements.
• Complete timely documentation as per policy.
• Supervise and provide clinical direction to nursing personnel.
• Perform on-call responsibilities as assigned.
