Job summary

Life Care Home Health Family administers nursing care in clients' homes, coordinating with an interdisciplinary team and families to sustain safe, effective care. The RN conducts initial and ongoing assessments, updates the Plan of Care, and ensures timely documentation and communication with care partners.

Applicants should have an RN license and at least 1 year in clinical care; home health experience is preferred.

Qualifications

• RN license in good standing with current CPR Certification.
• One (1) year RN experience in a clinical setting.
• Home health experience is preferred but not required.

Responsibilities

• Administer nursing care to home-bound clients in coordination with the care team.
• Develop, initiate, and revise Plan of Care (POC) to ensure quality care.
• Communicate with clients, families, and physicians; participate in case conferences.
• Apply infection-control practices and maintain required documentation.
• Provide on-call coverage and supervise nursing personnel as assigned.
