Job summary

Southwest General Home Health is seeking a Registered Nurse to provide one-on-one skilled nursing care in patients' homes across Avon, Avon Lake, and surrounding areas. You will lead care coordination, assess patients, and support families in the recovery journey.

The role offers a guaranteed base salary plus an uncapped bonus, mileage reimbursement, and flexible scheduling. RN licensure in OH is required, with preferred home care experience and a valid driver's license.

Qualifications

• Registered Nurse with current license in the state of employment.
• Minimum one-year experience as an RN in an acute care setting; home care experience preferred.

Responsibilities

• Plan of Care: observe and monitor patient conditions and develop an individualized care plan.
• Minimize Patient Risk: administer medications as prescribed and reduce hospital readmissions.
• Continuity of Care: coordinate multi-disciplinary care and manage admissions, transfers, and discharges.
• Family Educator/Advocate: educate patient and family using teach-back methods.
• Policies: complete clinical documentation following agency protocol and regulations.
• Rules and Regulations: follow agency policies and communicate schedule changes.
• Operations: attend in-service training and required meetings.
