Job summary

Alternate Solutions Health Network seeks an experienced Registered Nurse to provide skilled nursing care in patients' homes, coordinating care with physicians and a multidisciplinary team. The role emphasizes autonomy, timely visits, and accurate documentation to support positive patient outcomes.

You will administer medications, educate families, and manage plans of care while adhering to state regulations and agency policies. A valid license and reliable transportation are required.

Qualifications

• Registered Nurse with current license in the state of employment.
• Minimum one-year experience as an RN in an acute care setting.
• Home care experience preferred.

Responsibilities

• Plan of Care: observe and monitor patient conditions and coordinate care with the attending physician.
• Administer medications as prescribed and prioritize visits for high-risk patients.
• Maintain continuity of care across admissions, transfers, and discharges.
• Educate patients and families on disease processes and care plans.
• Complete clinical documentation per agency protocols and Medicare/ federal guidelines.
• Collaborate with interdisciplinary team to ensure optimal patient outcomes.
• Follow agency policies, procedures, and schedule communications with schedulers.
