Job summary

Interim HealthCare of Queen Creek & Gilbert is seeking a Home Health Registered Nurse (RN) for per diem roles across Queen Creek, Gilbert, Mesa, Tempe, Chandler, and surrounding East Valley communities. You will build trusted relationships in patients’ homes, exercising clinical autonomy to influence outcomes and schedule care that fits your life.

You will deliver skilled nursing in home settings, educate families, and coordinate with the care team.

Qualifications

• Arizona RN license active.
• Current CPR certification.
• 1+ year RN Home Health experience preferred.

Responsibilities

• Perform comprehensive patient assessments and contribute to care planning.
• Develop and update individualized plans of care aligned with physician orders and patient goals.
• Deliver skilled nursing services in the home setting.
• Educate patients and caregivers on conditions, treatments, medications, and self-care.
• Provide preventive, restorative, and rehabilitative nursing interventions.
• Monitor progress, identify changes in condition, and coordinate appropriate follow-up.
• Maintain timely, accurate clinical documentation.
• Collaborate with the interdisciplinary team to support continuity of care.
