Job summary

Interim HealthCare Inc. in Chandler, AZ seeks a Home Health Registered Nurse (RN) to provide compassionate home-based care with flexible scheduling and strong work-life balance. You’ll work 1:1 with patients, shape your day, and coordinate with a physician team.

Responsibilities include assessments, patient education, and guiding families on care plans, medications, and home safety. AZ RN license and CPR required; 1+ year home health experience preferred.

Qualifications

• Graduate of an accredited nursing program and AZ RN license.
• Minimum of 1 year nursing experience, ideally in home healthcare.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Clinical proficiency, strong communication skills and compassion.

Responsibilities

• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after discharge from a hospital, rehab or nursing facility.
• Work with a team of physicians and oversee LPNs, CNAs and aides providing patient care.
• Examine patients, perform assessments, document progress and report changes to their physician.
• Educate patients on their plan of care, prescribed medication, therapy, diet and exercise.
• Coach family members on the patient’s plan of care, medication and home safety.
• Coordinate discharge from services and ensure goals are met.
