You will operate at the top of your license, coordinating with an interdisciplinary team to ensure quality outcomes for patients in Warren, Clermont, Brown, Highland and Clinton Counties.

Qualifications

• Registered Nurse license in applicable state.
• Minimum one-year RN experience in acute care.
• Home care experience preferred.
• Valid driver's license and auto insurance in your name.

Responsibilities

• Provide skilled nursing care in patients' homes as directed by physician.
• Work autonomously and manage time efficiently.
• Coordinate care with interdisciplinary team.
• Educate patients and families using teach-back methods.
• Maintain clinical documentation following agency and Medicare guidelines.
