Job summary

The Lakes Home Care, Inc. is seeking a dedicated Registered Nurse to join our Home Health team in Florida. You will apply the nursing process to assess, plan, implement and evaluate patient care under the direction of the Director of Clinical Services.

Qualifications

• Current RN license for the state of practice.
• Current Health Certificate per facility RN contract or state regulation.
• Current PPD or Chest X-Ray.
• Current BLS card.
• One year RN experience preferred.

Responsibilities

• Utilizes the nursing process to assess, plan, implement and evaluate patient care.
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
• Collects, analyzes, and interprets data and documents nursing diagnoses.
• Documents the patient’s plan of care with identified nursing diagnoses, expected outcomes, and interventions.
• Performs interventions according to priorities, plan of care, and policies.
• Revises the plan of care according to evaluation and changes in medical plan.
• Uses clinical judgment in evaluation activities to meet patient care needs of the assigned unit/floor.
• Other RN duties as assigned.
