Elara Caring is seeking a Registered Nurse Home Health to deliver high-quality, compassionate care in patients’ homes within the Grand Rapids area. You will assess and plan patient care, coordinate with the interdisciplinary team, and ensure HIPAA compliance while supporting autonomy in scheduling and visits.

The role requires a current RN license, 1+ year of clinical nursing experience, and the ability to travel up to 50% using a personal vehicle.

Qualifikationen

• Current, unrestricted state RN license.
• 1+ year experience in a clinical care setting as a nurse.
• Willingness to travel within branch/office coverage area (up to 50%).
• Must be able to lift up to 50 pounds and assist patient transfers.
• Must have dependable vehicle, valid driver's license, and current auto insurance.

Aufgaben

• Assess caseload for home health patients and families to identify needs.
• Plan, implement, and evaluate the plan of care.
• Make initial evaluations and ongoing re‑evaluations during visits.
• Perform follow‑up visits per patient and facility needs.
• Communicate findings to Clinical Supervisor, physician, and care team.
