Job summary

One Home Health LLC is seeking an experienced Home Health Registered Nurse (RN) to join our team serving areas on Chicago's North Side including Northbrook. The role involves managing a caseload, delivering skilled home visits, and making independent clinical decisions to ensure high-quality patient outcomes.

The position is Part-Time, PRN/Per Diem with opportunities for growth and pay ranging from $50.00 to $65.00 per hour depending on experience.

Qualifications

• Active Illinois RN license is required.
• CPR/BLS certification required.
• Strong clinical assessment and critical thinking skills expected.
• Excellent time management, organizational and communication abilities.

Responsibilities

• Manage a caseload of home health patients with skilled nursing visits and care plan updates.
• Perform admissions, recertifications, resumptions of care, and discharges.
• Complete accurate OASIS assessments and documentation.
• Coordinate care with physicians, therapists, caregivers, and interdisciplinary team members.
• Educate patients and families on disease management, medications and home safety.
• Monitor patient conditions and respond to health status changes.
• Administer medications and skilled nursing interventions as ordered.
• Maintain documentation meeting Medicare, State of Illinois, and agency standards.
