Job summary

Sonder Healthcare is seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes throughout Natomas, West Sacramento, and Downtown Sacramento, CA. This per diem position offers schedule flexibility and autonomy in the field.

You will perform assessments, develop care plans, administer treatments and medications, educate patients and caregivers, coordinate with physicians and the care team, and maintain timely documentation.

Qualifications

• Active California RN license required.
• Minimum of three years nursing experience.
• Home health experience required.
• Current BLS certification (American Heart Association).
• Valid driver’s license with reliable transportation.
• Strong clinical, documentation, and communication skills.

Responsibilities
